Authorization to Release Information Form
Use this form to authorize the Financial Aid Office to discuss your financial aid information with a parent/spouse/significant other. This form must be printed, filled out, and either mailed, faxed or hand-delivered to our office.
Goal Form
This form can be used to temporarily take the place of a Student Education Plan (SEP) for:
  • First-time freshman (NEVER attended college before) who have missed all orientation sessions
  • Out-of-area transfer student
  • Student officially admitted to the Nursing Program
  • Student officially admitted to the Psych Tech Program

If the above categories do not apply, you must make an appointment with the Counseling Department and complete a SEP. Please call (805) 546-3138 to make that appointment.
